Uniform Return/Keeping for Track

It’s that time of year…the season is winding down. Saaaaaad. So, it is time to return uniforms!
You have two…well, three options, but two seem much better than the third:
If you are going to run on the track team in 2023, you can download the form below, fill out the form and return it to Coach Flatow or to Mrs. Flatow. You can return a paper copy or email a scanned copy to coach, your choice. (Only do this if you are sure you are going to join the track team; if you do not join, you will be charged a $250 ASB delinquency.)
To return your uniform, meet Mrs. Flatow in the student parking lot on Monday, November 14 at 4:10pm and return your complete uniform washed and clean. Please make sure the numbers you were issued match the numbers you have. If you have lost any piece of your uniform, or anything is ripped, please bring a check for ($50 per item lost or damaged) made out to Monta Vista ASB.
The third option, not preferred, is that you are charged $250 for not returning your uniform. We prefer option one or two!
