Our first day of track practice for the 2015 season is right around the corner–we start practice on Monday, February 2 at 3:20pm out on the track!

Don’t forget all your paperwork (if you have already competed in a sport this year, most of this paperwork you can get from Marie, the trainer):

Physical Forms A and B (with doctor’s signature)

Parent Waiver (including doctor’s signature and insurance information)

Code of Conduct acknowledgement (signature page only is OK)

Participation Agreement

Concussion Information acknowledgement (signature page only is OK)

Boosters Membership

That is at least seven pieces of paper!  All of these forms are available at the MVHS athletics web site.

Also, if you know that you are going to have conflicts with any practices or meets this season, download and use the excused absence request form–this does not guarantee that absence will be excused, however it is more likely that your coach will approve a request in advance–that shows forethought on your part.

And lastly, all seniors, please fill out the Seniors form which is downloadable below, and if you need PE credit to graduate you MUST have a parent sign the form also.  We want there to be complete clarity this season, and going forward, about expectations for anyone looking for PE credit and the repercussions if you are cut from the Track & Field team.
